Codex switches to token-based API pricing for all users

Codex is moving its credit system to token-based API pricing so credits map directly to token usage.

Existing Plus, Pro and Enterprise/Edu customers will be migrated to the new rates in the coming weeks.

The change replaces message-based credit estimates with a clearer token-based calculation that shows how input, cached input, and output affect consumption.
